Reason for IPI's under the Slats

I hear this IPI was started due to a hydo leak from one of the slat acuators that was set on fire by a shorted 28vdc wire, hence the IPI's to help keep it from happening again. From what I was told it was a Moody Hawg that caught fire inflight with no way to put it out some years ago. Any truth to the rumor?

422, that would have been my jet 157 ( I was the CC) and yes that's what the board said. To tell you the truth I have never belived this because, she wasn't leaking when I did the preflight, it flew once came back and hotpited (no one saw any leaks) then crashed on the next flight.

The pilot said he did a gun pass, pulled-up and backed left, and when he looked over the left wing was on fire. Leveled-of looked over again and the whole inboard wing was burning and he had a full fuel tank so out he went.

BTW the aircraft cut a road in half and struck the ground on the left wing...there was not much left of the wing to even investigate!

and unbeknowing to dice, he had brought in his replacement acft......the look on his face when they told him......but that wasnt as good as the slick sleever on his 1st launch has the plane crash.......It wasnt funny then of course, but looking back on it now

Sounded odd, but if I was the pilot and saw my wing full of fuel on fire I propably would have pulled the egress handles too. Not like we have fire bottles in the wings. Was just wondering if there were differing opinions than that of the crash investigation. Must have been a rough crash site with a fire previous to the one on the ground from the full fuel load.

Yep, not much left of her! In fact the Ops guys put the gun barrels outside the ops building because they were so twisted they looked like modron art! I also remember we had a commanders call in the hanger they put all the parts in for the investigation about three months later. There was a large box in the corner with parts in it, someone wrote on the side

Well isnt there also an IP because there is ballistic foam under the panels ? All panels that have it are supposed to be IP'd.

There are plenty of panels with no IPI that have foam, such as W-1, W-2, the uarrsi drain panel, fwd and aft main pump panels...(can't remember the #'s on those last ones)

Sounds more like a fuel leak than Hyd. fluid....that stuff is kinda tough to light.....unless you stuffed the workcards you were reading step by step in there to soak up the fluid. Hey hog166....has anyone told you today you suck?

Kinda what I thought about fuel vs. hydro but thats why I asked the question. It would have to be one hell of a short to burn hot enough to light a hydro leak. I didn't even know that hydro fuild was that volitale once lit either. Wonder what kinds of tests were done by the board.

My dads a retired CMSGT, workd 26 years in the fire department...after 127 went down & the reason was released I asked him...he said hyd fluid is very flamable in vapor form....who knows...

Actually Dice's jet wasn't the only one that happened to and was lost.
And it was determined the cause was from a chaffed wire shorting on a hydraulic line. It burnt a hole through the tube and the pressurized hydro fluid was ignited.
There were TCTO's that came out because of it. One in particular added posts and clamps to keep the wire bundles away from the lines. One place that is nearly impossible to avoid is the area where the lines and wire bundles transition from the wing to the fuselage as you may know that is a snakes nest. That area is the focus of the IPI.

Let me dig through my stuff.
As I recall there were also mirror TOPS added to the 24MS & 29MS that highlighted those areas.

Ok, the TCTO was 1A-10-1392 Prevention of Wire Bundle Chafing Behind Leading Edge Wing Panels on A-10A/OA-10A.
I tried to find the other aircraft that were involved, but a quick look through my stuff didn't bring it up. I know of one other for sure because one of our pilots (45th FS), was the wingman on that incident. It seems to me it was either 442nd jet when they were still at RG or a Wisconson Guard jet when Madison had A-10s.
