Remember the aircraft,I think it was United Airlines,that lost all hydraulic pressure in both systems after it lost an engine. The fan disk came off and cut through the lines. Probably the same cause.

According to the TV program, Seconds From Disaster, the fan failure on the airliner was due to a defect in the hub of the fan disk. The fan disk had been inspected a few months before the incident, and the crack should have been visible to the inspector, but it was missed. Eventually, due to the forces of operation, the crack finally became large enough to allow the hub to break, and the disk came apart, taking the hydraulic lines with it.
